Blizzard confirmed a fix for the chests dropping guaranteed Greater Affix items is coming soon, most likely later today.
Spoils of Greater Equipment in Infernal Hordes are not currently dropping any Greater Affix items and it seems the final Season 5 journey cache is affected by the same bug.
Fortunately, Adam Fletcher confirmed Blizzard is working on a fix for the issue.
