A bug in Season 5 is currently affecting the Spoils of Greater Equipment in Infernal Hordes, preventing them from awarding items with guaranteed Greater Affixes, even though they should for 60 Aether.
There is no guaranteed GA drop from the 60 Aether chest despite the tooltip saying so.

Infernal Hordes Tier 6 and below are bugged, according to player reports, but Tier 7 and above work and grants GA drops.
Hopefully, this bug will be addressed in the next round of hotfixes.
